- Oral exams (every six months)
- Dental cleanings (every six months)
- Bitewing x-rays (three every 12 months)
- Fluoride (every six months)
Treatments
- Fillings--one per 24 months
- Crowns--one per tooth every 60 months
- Root Canal--one per tooth per lifetime
- Extractions--one per lifetime
- Periodontics--one per 12 or 24 months, same quadrant, depending on age restrictions
- Dentures--one per tooth, per 96 months
- Implants--one per tooth, per lifetime
