Find/Change Your Dentist

Do you want to find a new dentist or change your Main Dentist or Dental Home? You’re in the right place. 

Use the Find a Dentist tool to search for dentists. Click the View Details button to learn more about a provider.

To choose a new Main Dentist or Dental Home, click on the Set as Main Dentist button. Then follow the instructions to complete the change. You will need the Member or Medicaid ID, the Member phone or last four numbers of the Member’s SSN, and Member DOB ready.
