Senior Whole Health by Molina Healthcare

DentaQuest provides dental coverage to Medicaid-eligible adults in Massachusetts who are members of Senior Whole Health by Molina Healthcare.

Prevention

• Oral exams (one every six months) 
• Dental x-rays (once per 36 months per patient)
• Teeth cleaning (one every six months)
• Fluoride (two applications per 12 months per patient) 

Treatment

• Fillings (once per 12 months per patient per tooth per surface)
• Crowns (one per tooth every 60 months)
• Root Canal (one per tooth per lifetime)
• Extractions (teeth removal) (one per lifetime per patient per tooth)
• Periodontics (one per 36 months per patient per quadrant)
• Dentures (one per lifetime per patient)
