TENNCARE PROVIDERS

Fluoride Varnish and Dental Screening Program

Reduce dental caries in Tennessee children and help improve oral health throughout their lifetimes when you participate in the TennCare Non-Traditional Oral Screening and Fluoride Varnish (NTFV) in the Medical Home Program.

ENROLL IN THE NTFV PROGRAM

  1. Complete an enrollment form for each provider.
  2. Email completed form to Non-TraditionalMedicalFluorideVarnish@greatdentalplans.com.
  3. DentaQuest will validate your eligibility and email you the complete enrollment packet.

 

Enrollment takes approximately 30 business days to complete, once all required documents and information are received and verified. A welcome toolkit and resources will be sent to each clinic.
