Most providers can join our network in one of two ways:

Complete and submit a DentaQuest credentialing application online.
Sign up with The Council for Affordable Quality Healthcare (CAQH) and maintain your credentialing information in one place. There is no cost and online registration is fast and easy.

We encourage you to complete your credentialing application online. However, if you prefer to use a paper application to enroll by mail, please call a DentaQuest provider relations representative.

Enroll in CHAMPS to serve Michigan Medicaid Members

All providers who serve Michigan Medicaid beneficiaries must enroll in CHAMPS to receive payment from dental health plans. You must revalidate your information with CHAMPS every 5 years.

Visit the CHAMPS website.
Select "Register for MILogin Account for Access to CHAMPS"
Select MILogin Instructions
Apply for a user name and password, and then subscribe to the CHAMPS link.

Ready to Recredential?

Thank you for participating in the DentaQuest network. In accordance with National Committee for Quality Assurance (NCQA) guidelines, providers are required to undergo recredentialing every three years.
