AzAHP has contracted with Aperture Credentialing for primary source verification (PSV) services. Any correspondence from Aperture on behalf of DentaQuest is legitimate and vital to the timely completion of your initial credentialing application.

Through the Council for Affordable Quality Care (CAQH)

CAQH has created a single credentialing application and online database with the necessary information for insurance companies to credential a provider. This allows you to submit and maintain your credentialing information in one place, rather than filing with multiple organizations. There is no cost to file an application with CAQH.
