Limitations or Restrictions

Eligible members have DentaQuest as their Florida Healthy Kids dental plan administrator.
Members who qualify will be eligible for the gift card once per lifetime.
Florida Healthy Kids (CHIP) members ages 6-14 must meet the qualifying dental service - receive sealants.
