When can you expect your gift card?
If you qualify, you can expect to receive your gift card in the mail 12 weeks after you submit the redemption form.
*Gift Card can be used to purchase:
- Oral health supplies such as toothbrushes, toothpaste, dental floss and mouthwash.
- Baby care such as diapers, bottles, and formula.
- School supplies and clothes.
Member must submit the redemption request to DentaQuest within 60 days of the date of service. Treatments must be provided by an in-network provider while the Member is enrolled with DentaQuest.
