If you or your child is a DentaQuest Health First Colorado (Colorado's Medicaid program) or Child Health Plan Plus (CHP+) member and received care on or after January 1, 2025, you may qualify for one of the following:
- For Health First Colorado Members—all ages.
- For CHP+ Members—all ages.
- Complete the Oral Health Risk Screening form within the first 90 days of enrollment and submit the redemption request.
- Limit of one gift card per Member per lifetime.
- For Health First Colorado Members—all ages.
- For CHP+ Members—all ages.
- Receive a preventive dental service within the first 90 days of enrollment with DentaQuest and submit the redemption request within the 60 days of the date of service.
- Limit of one gift card per Member per lifetime.
- For Health First Colorado Members—ages 6-14 years.
- For CHP+ Members—ages 6-14 years.
- Receive a sealant treatment on any of the first or second molars.
- Limit of one gift card per Member per lifetime.
- For Health First Colorado Members—all Child Members.
- For CHP+ Members—all Child Members.
- Receive two preventive dental services within a 12-month period.
- Limit of one gift card per within a lifetime.
- For Health First Colorado Members—Adult Members.
- For CHP+ Members—Adult Members.
- Complete an online assessment and view a video on opioid safety and pain management after a tooth extraction.
- Limit of one gift card per Member per lifetime.

When can you expect your gift card?
If you qualify, you can expect to receive your gift card in the mail 12 weeks after you submit the redemption form.
*Gift Card can be used to purchase:
- Oral health supplies such as toothbrushes, toothpaste, dental floss and mouthwash.
- Baby care such as diapers, bottles, and formula.
- School supplies and clothes.
Member must submit the redemption request to DentaQuest within 60 days of the date of service. Treatments must be provided by an in-network provider while the Member is enrolled with DentaQuest.
