Equality Health Foundation’s mission is to break down the barriers in access to whole-person care and level the health care playing field in historically marginalized and underserved communities. The Foundation’s goal is to eliminate health disparities by addressing social determinants of health, while accelerating health education so that everyone in the community has an opportunity to lead healthier and longer lives. Since its inception in 2019, the Foundation has helped nearly 90,000 people across Arizona, Texas and Tennessee.

With headquarters in Arizona, Equality Health Foundation also serves Texas, Louisiana, Tennessee and Virginia, and will add North Carolina next. Through health festivals and in collaboration with community partners, the Foundation provides access to health screenings and immunizations, health education and awareness, resource connections, referrals to health care providers, and food distributions for those who are from underserved and under-resourced communities.

The Foundation’s flagship program is a large health festival that has successfully been duplicated and customized for the populations in each state, meeting their cultural and health equity needs. During the 7th annual healthy fall festival & turkey giveaway in Phoenix, 5,000 attendees were reached, 12,000 pounds of produce distributed, 2,000 meals served, 1,500 healthy bags donated, 2,800 medical screenings and immunizations conducted, and 1,500 turkeys given to families in need. This signature festival serves as a model for other initiatives, and it has expanded to Texas and Tennessee.

The Foundation credits this successful, replicable community engagement to its three-prong approach:

  1. convene diverse stakeholders to identify community and social care needs;
  2. collaborate with public-private partners to address disparity gaps and remove barriers to good health and well-being; and
  3. innovate and co-create solutions with a diverse alliance of community stakeholders to solve for the social determinants of health.
