EyeQuest Vision Plan Information

Vision providers can temporarily provide telehealth services to most members according to national or state guidelines currently in effect.
 
EyeQuest Members: Please call your vision provider or any provider in our network (no referral required) to learn more about your options for remote eye care. If you need help locating a vision provider who offers telehealth, please call the customer service number on your ID Card.

EyeQuest Network Providers: Please check the Provider Web Portal about reimbursable telehealth codes, consult your state-specific mandates or contact EyeQuestProviderServices@dentaquest.com.
