Limitations or Restrictions
- Eligible members have DentaQuest as their Florida Medicaid dental plan administrator.
- Members who qualify will be eligible for the gift card once per lifetime.
- Medicaid members ages 0-20 must meet the qualifying dental service within 180 days of enrollment.
