If your child is a DentaQuest CHIP or Medicaid Member and received care on or after September 1, 2023, you may qualify for one of the following:
For ages 12 months - 2 years
Kit includes a durable zippered backpack, infant toothbrush, toothpaste, brushing chart and stickers.- Also, adult oral health kit is included for the Member’s parent/guardian with educational materials, spinning toothbrush, and a timer to encourage 2-minute brushing and floss.
- For Members ages 12 months to 2 years old who get a preventive dental service within the first 180 days of enrollment and return the Value-Added Services form to DentaQuest.
- Limit of one kit per Member per lifetime
For ages 3 - 8
- Kit includes a durable zippered backpack, toothbrush, toothpaste, brushing chart and stickers.
- Plus, adult oral health kit is included for the Member’s parent/guardian with educational materials, spinning toothbrush, and a timer to encourage 2-minute brushing and floss.
- For Members aged 3 years old to 8 years old who get a preventive dental service within the first 180 days of enrollment and return the Value-Added Services form to DentaQuest.
- Limit of one kit per Member per lifetime.
For ages 9 and up
- Kit includes a durable zippered backpack, spinning toothbrush, toothpaste, brushing chart and stickers.
- Also, adult oral health kit is included for the Member’s parent/guardian with educational materials, spinning toothbrush, and a timer to encourage 2-minute brushing and floss.
- For Members aged 9 years or older who get a preventive dental service within the first 180 days of enrollment and return the Value-Added Services form to DentaQuest.
- Limit of one kit per Member per lifetime.
- For DentaQuest Medicaid Members aged 12 months through 20 years old.
- For DentaQuest CHIP Members aged 12 months through 18 years old.
- Must get a qualifying Texas Health Steps dental checkup.
- Limit of one gift card per Member per lifetime.
- For DentaQuest Medicaid or CHIP Members who complete a follow-up visit with their Main Dentist within 30 days if they had to visit the emergency room for a dental-related issue.
- Complete educational quiz about emergency room dental visits and submit to DentaQuest.
- Limit of one gift card per Member per lifetime.
- For DentaQuest Medicaid Members ages 20 years old and younger.
- For DentaQuest CHIP Members ages 18 years old and younger.
- Download and register for the DentaQuest Mobile App within 30 days of enrollment with DentaQuest.
- Limit of one gift card per household registration per lifetime.
- For DentaQuest Medicaid Members ages 20 years old and younger.
- For DentaQuest CHIP Members ages 18 years old and younger.
- Members who complete the risk assessment, qualify, and enroll in the DentaQuest Case Management Program.
- Limit of one gift card per family per lifetime.
- For DentaQuest Medicaid Members ages 20 years old and younger.
- For DentaQuest CHIP Members ages 18 years old and younger.
- Members must receive two fluoride treatments within 12 months.
- Limit one gift card per Member per lifetime.
- For DentaQuest Medicaid or CHIP Members ages 9 and under who receive sealant treatment on all four first molars in a 12-month period.
- Limit one gift card per Member per lifetime.
- For DentaQuest Medicaid or CHIP Members ages 10 years old to 13 years old who receive sealant treatment on all four second molars in a 12-month period.
- Limit one per Member per lifetime.
- For DentaQuest Medicaid or CHIP Members ages 14 years old who receive sealant treatment on all four second molars in a 12-month period.
- Limit one per Member per lifetime.
- DentaQuest Medicaid or CHIP Members who receive four fluoride treatments (2 per year), four sealants (2 per year), and four oral evaluations (2 per year) within a 24-month period. That’s a total of up to $100 in rewards for receiving important preventative services!
- For Members ages 3 years old to 10 years old.
- Limit one gift card per Member per lifetime.

Complete the Value-Added Services form below and submit it to DentaQuest.
When can you expect your reward?
If you qualify, you can expect to receive your reward in the mail 2-4 weeks after you submit the Value-Added Services form.
When you claim a gift card, you will have the option of choosing a plastic or digital version. The plastic gift card will be sent to you by mail. The digital gift card will be sent to you by email. You will need to provide an email address if you choose a digital gift card.
*Gift Card can be used to purchase:
- Oral health supplies such as toothbrushes, toothpaste, floss and mouthwash.
- Baby care such as diapers, bottles, and formula.
- School supplies and clothes.
**Member must return the Value-Added Services form to DentaQuest within 60 days of the date of service. Treatments must be provided by an in-network provider while the Member is enrolled with DentaQuest.
