All it takes is a few simple steps:

  • Call our Sales Department at 877-508-9582 or email us at DQBrokerSalesSupport@DentaQuest.com.
  • A sales executive will contact you.
  • A Broker's Agreement will be sent to you.
  • Return the signed agreement, along with a copy of your current Broker's license in each state you intend to sell in, the Licensing Information Form and a W-9 form to your sales executive
  • The sales executive will show you how to offer your Individual clients dental plans to meet their needs.
