This value plan covers preventive and diagnostic services, including exams, cleanings and x-rays with additional coverage for basic restorative services such as crown repair and fillings.

EPO Plan Availability: Florida, Indiana, Louisiana, Pennsylvania and Tennessee
Our most comprehensive plan includes coverage for preventive services, basic restorative services, and complex services such as root canals and oral surgery.

This comprehensive plan features coverage for preventive and diagnostic services at no cost, basic and complex services as well as orthodontia services for both children and adults. High benefit maximum of $2,000 per member, with a separate orthodontia maximum of $1,500 per member.

This elite plan has no waiting periods for all covered services, with the exception of orthodontia.
